I'm i'm including an incipit of a piece into a paper, how do I list the score in my bibliography? Do I just use the web address?

This would of course depend on the format of your list and how detailed it is supposed to be.
The official owner of the copyright of the scores is the company behind Piano Street, Op 111 Productions, so I would suggest something along the lines of "Piano Street (wvvw.pianostreet.com) by Op 111 Productions".
Except for that you could pick as much details as you need from here:

Where is PS getting the material that becomes the scores though? I've wondered about that a lot. Is it from a manuscript? An older, out of copyright edition?
