The local church bells.
Cars/motos with a defective escapement.
Pianos out of tune.
Beginners trying to play the violin, flute, Sax, Oboe, etc.
Babies screaming.
Hydraulic stone crushers.
Alarming noises from machinery (computers, cars, TV, etc.) that may signal a problem that will cost a fortune to fix.

People moaning about things they have no right to moan about.
-
The sound of a room full of loads of people, none of which I know.

Xenakis' "Music"
Those really, really, loud sirens on ambulances. As if we can't see them with their lights that could give anyone who looks straight at them for 10 seconds an epileptic seizure.
Pianos out of tune.
Ravel loved out of tune pianos :). They give a very interesting feel to a piece, makes it sound less mechanical and more mysterious.

I hate the sound of the surgeon saying "whoops!" just as you go unconscious.
I also hate the sound of people vomiting...urgh! :P

Boulez
Children crying in a restaurant
Cell phones going off during a performance
Sawing metal by hand
People mispronouncing etcetera, actually, people mispronouncing anything
a seriously fumbled passage in any of my performances
honking of car horns for no apparent reason
buzzing, electronic alarm clocks
any particularly obnoxious telephone
the stupid sounding music that some people's cell phones make when ringing

I guess strictly speaking pianos are by definition out of tune. (equal temperament and all that). But I was referring to terminal cases. I once went to student's house where the middle C, the D and the E all sounded exactly the same C#. And these were the good notes. :P

Fur Elise ringing tone! ;D
-
The sound of the rubber gum boots brought right into the house and squeeking squeeking...just as bad as the chalkboard.
-
the cat next door going INSANE in the middle of the night and making weird loud noises and jumping on the tin roof (????????????)
the music they play when out clubbing (oh woohoo i get to listen to it tonight)
the sound of my voice when it's sightsinging, actually singing in general, it's quite painful
drunken teenagers walking past our house in the middle of the night making a stupid amount of noise and emphasising the fact that they're 'drunk'
-
Approaching, heavy, twigg and stick busting footsteps, in the middle of the night, in the middle of a forest
Creaks and knocks, when I am alone in a strange house, especially after dark (I have done a bit of house-sitting as part-time work in the past)
The creaks an old boat makes as it sits upon the water and is rocked by the swells of the sea
The sound that a mulch pile full of maggots makes
A little dog yapping all day long (or any dog barking all day long)
People arguing with harshness and venom in their tone (especially when the voice is my own!)
-
The sound of George Bush's voice!!! (Yes I know Bernhard already said this but I thought of it before I read his post and I thought it was well worth repeating)
-
those damned wild dogs that keep barking at night just outside my back yard.
-
All unmusical high pitched sounds!
All loud unmusical sounds.
All very loud sounds.
