Heart and Soul

Does anyone have the notes for Heart and Soul written out? I was thinking of transposing this to guitar as something to show-off with.

The chord progression is C, Am, F, G. Start on C, and I'm sure you'll be able to figure out the tune, it's not particularly difficult. If you don't know it, listen to it, it's a Hoagy Carmichael tune I think.
