Here is the notation. I need help on the treble. Any fingering suggestions?

That looks rather straightforward to me. So, you must be having a specific problem. Care to elaborate?

How about:
(1/4)
(2/5)
(1/3) or (2/4)

which piece is this I can't place it.

I don't know if this is weird or not
Hopefully there isn't a need for legato with the thumb movement I am proposing
1,3
1,4
3,5

at first I was doing
1,3
2,4
3,5

which is kindof a weird stretch for me but doable.

Then i was doing
1,4
1,4
3,5

but I do like the suggested 1,3 1,4 3,5 better than that last one I listed.

I guess it is pretty straightforward for you guys not me though. Whatever i finally feel comfortable with I'll probably pick.  No legato is needed.  Now I have come to a harder part of the piece and probably the last of my questions on it.  This is a "jazzy" piece. I'm sorry if this print out is blurry but I need help on the middle bar of this:

Do i slide my thumb down from e flat, to e to f? How do I play this?

It is a little blurry for me to see even with the computer magnifier. By going by what you wrote out, I think that your suggestion is fine. You may also want to try using finger 5 on the A and try to work a combination with fingers 1 and 2. I would have given a suggested fingering but I can't see the specific notes. If you write out the notes in your next post then I will provide one for you.  :)

E flat: 1
E: 2
F: 1
(A flat: 3)

This is how I would finger it.
