You mean the arpeggiated chords in both hands? You just practice them H.S. and fit them together so you hit the D# and the B at same time. Honestly, I think I play the notes together until the last 3 notes in RH which I do along with 2 notes in LH, as a 3 against 2.

The lead-in in the previous bars is very important, and hard! Be sure to emphasize those rising chromatic bass notes, and in bar 43, emphasize the left hand B, then the C#, then the right hand D# must sound, even though in a decrescendo. Continue the decresc. through the arpeggio and leave it "hanging" in the air for a second before going on to the "a tempo."
