sight reading skill

Most of my skill in sight reading has simply come from playing through pieces. It works, as I'm a reasonable sight reader, but has anyone tried any exercises, books, or courses that you found very useful in taking sight reading to a higher level?

The thread mentions Richman's book, which I've heard rave reviews about.
Accompanying is one of the best (and scariest) ways to improve sightreading ability.

There is a series of workbooks available called 'Improve Your Sight-Reading'. These workbooks encourage analysis of the music, and they teach some sight reading techniques.
