It's in an album of Yamamoto transcriptions that's been out of print for a few years. The album is listed by Amazon Japan at https://www.amazon.co.jp/gp/switch-language/product/4276438829/ref=dp_change_lang?ie=UTF8&language=en_JP

Volume 1 is still available. If anybody has a copy of volume 2 I would love a scan of Angel too. It features on a recent CD by Kathryn Stott.
