I can try to do it by ear, maybe just some of it and another person can finish it!
So the first not is just an F in the bass clef. Than you have an F major chord 6, so that would be an F major chord with the F note on top. Than I believe another F but closer to middle C. Than it is a C major chord. Than an Eb. Than a C I think. Than I think the last chord before the singing consists of G on the bottom, C in the middle, and the D right above the C. I think it just repeats after that!
Okay at the :22 mark you have an Ab 6 chord, so a C, Eb in the middle and Ab on the top. Than a Bb 6/4 chord = F on the bottome, Bb in the middle, and a D on top. Than it start to repeat the beginning again
An I think that is it! I hope that is right.

Thanks Natalie for replying. But could you explain it a bit more please if that is possible. I have only been playing piano for a couple of weeks and haven't got to grips with all the terminology. Like what do you mean by G on the bottom? do you mean on the left side of middle C?

Sure. For the first chord, you will take your thumb and play an A, with your pointer play the C, and with your pinky finger play the F. Here is a picture of middle C on the piano:

Gosh that is a huge link! Sorry. I think it is the F below the C, if that makes sense! A natural C major chord is played with the thumb playing the C, the pointer playing the E, and the pinky finger playing the G. After just the C note is played after the C chord, the next chord is played by taking your thumb and putting is on the G, you ring finger on the C, and your pinky finger on the D.
At the :22 mark, the Ab chord is played with your thumb playing the C, your pointer playing the Eb, and your thumb playing the Ab. Then the Bb chord in the Thumb playing the F, the pointer playing the Bb, and your thumb playing the D.
All of these chords are played nearest middle C.
