Just curious what you use.

One of the little handheld recorders if I want something quick and easy.

A little portable set up if I want more professional quality.

I just use my mobile phone, nokia N8, from 4 years ago. It picks up the different dynamics of my playing reasonably well, plus it can record 720p videos at 30 fps.

For audio: Studio Projects B1 mics, Edirol UA-25. 

For video: a digital camera that does video.  Record audio using internal mic (only used for sync).

Record video and audio separately.  Make sure to slate each take to make organization easier in editing.  Make sure there is an audiable "clap" to make sync easier.  Combine both in editing, and use the internal video mic track to align the studio mics track to the video.  Mute the internal video mic track.
