a good & easy to understand book to begin with

Hi,
I would like to know if there is a simple and good book to learn playing music from notes.
I have been playing piano for 15 years by ear (some classical and a little bit of Jazz) but now I feel the need to learn notes,bits (rythm) and some theory.
