How difficult is it? Many tremolos for the left hand?

Supremely. It is one if the most difficult transcriptions /realizations or solo piano i have seen.  All sorts of acrobatics involved.

Yes  so they would be the least of your worries. 
This is probably one of the reasons that the recording I own is the or one of the only ones of it thatched been done.  I almost made the foolish decision to take it oblast year but I realized the hundreds o hours it would take me aty current level and I cannot dedicate that muh time and focus to a single work at this time.

Awesome piece though.  I love the third movement the most
