First attempt at writing a jazz piece..

Hi guys,
I havent participated in the forum for quite a while as I was working on a first jazz piece. It is supposed to be a romance for piano and orchestra (strings, horn and flute) but I'm not at all sure about the structure yet. Also jazz is completely new to me and I didn't really learn any jazz theory first but just listened to jazz musicians I like.
Please tell me, even though it is a short sample, what you think of it and what I could improve. Also, I included a sheet as I want to ask you guys about the notation for the piano. Since I'm classicaly trained, it looks really messy but is this okay. But my priority to receive advice about is the harmony, I'm just writing from ear and try out certain chords. Should I learn jazz harmony at least a little bit maybe?
