Taking piano exam

Mind goes blank during piano lesson due to nervousness and I wonder how I can keep calm during my practical exam ! I experienced 'blank mind' during my violin exam but managed to keep calm and continue playing although I did screw up abit here and there but this time round, it is the piano and once I screw up one note, I will panick and completely lost it. This happens many times during my piano lessons even when I have been practising for hours before my lesson and I can foresee the same sh*t will happen during my exam. How to deal with it ?
