identify this piece, please

Hello,
recently I heard the following piece (attached). I have absolutely no information about this piece: neither the composer nor the date of composition.

Having the melody in my mind, I typed it into a music notation software and exported it as mp3 and pdf (see attachments).

I'd welcome your help. Thank you in advance :-)
