i thought these people were very rare.  unless created by plastic surgery.  i think then, the doctor carries a certain culpability if the person definately had a gender before undergoing a radical change.  i don't really know about undecided cases.  i think for men with boobs - certain plastic surgeons go the simpler route and take them off instead of creating a whole new woman.

If it were obvious then I suspect it'd be like being mixed race, you'd be more likely to experience everyone's racism rather than being accepted by both and avoiding the prejudices.
