I have read that the worst sound for most people is the sound of someone vomitting.  The amount of splatter sound having a significant impact on the distastefulness.

well, there's one other sound.  those bug killer plug-in's.  if you have fairly acute hearing- you can go crazy from the high pitched (supposedly out of human range of hearing).  i think it could be a form of mental torture.  surprisingly they have them on new cell phones so that students (teenagers with more acute hearing) can hear their cell phones go off.

agreed about the vomit being a tie.  although, i still think beginning violin players take the cake.

one of the worst sounds for me is my dad eating.  i can't stand it.  it's very strange -- his jaw pops and cracks when he chews and it's gross.

Babies crying. It stabs right through my brain. But there are biological reasons for that, aren't there?

And the sound of someone scratching their head (assuming they have hair) is guaranteed to make my teeth itch.

There is a distinctive sound of an instrument (any kind) hitting the floor that is very... unpleasant.

That's it.... Hear the grand piano tipping off the stage and hitting the floor.  Hear the sound of an upright clunking back down all those stairs.  Hideous!

Small woodwinds have a special clicking sound when they hit the floor and bend keys.

Brass have a longer reverb time with this unique metal bending crunch sound.
