Certification/Letter of recommendation

I am supposed to participate in a competition where they require (as in their website)
1 certification issued by the teacher or the institute where the candidate is taking his / her piano courses, or a C.V.
What should I write in that or what should my teacher write in it? and what is the general form of it? Most probably I will just write it and let my teacher just sign it
If I will send a CV, what should I write in it? I almost have no musical qualifications, I didn't study in a conservatory and I didn't win any competition before. I only have grade 8 ABRSM.
Also, the competition is in Morocco (a french-speaking country), so should i write it in french or it's not necessary?
Can anyone help?? I have no idea what to write in that!

Think up all performances, and music related activities you have gone through and list them.
