Need your help for a small inquiry for pianists :)

You might remember me from this topic (https://www.pianostreet.com/smf/index.php/topic,29631.msg342686.html#msg342686), a discussion about how important a pianist thinks it is to know his/her instrument.
Now for my final thesis I need to do a small inquiry which I'm sending out to as much performers/pianists as possible. So if those of you who want to could help me out and fill this in, I would really appreciate it. My e-mail address is inside the file.

Apparently it's not possible to attach .doc files, so I had to upload it to mediafire.
