Hello - Chords and Notes

hello, new guy here, wanting to learn :)

Can someone help me here:

What I want to know is, am I playing just the note D next to middle C?

because it's located here:

Or am I playing G, A#, D which is the G minor Chord?

But then again, why is it telling me to play it in G minor? If the ledger lines in Figure 3 tell me that it is supposed to be next to middle C, why is the Gm necessary for it to be above the note?
