...so how do I practice?

I know this is a frequently asked question. However, despite my constant attempts to reach ground (be it through research on similar topics in this forum, discussion with my teacher, etc) level with this subject, I'm still left somewhat unsettled.
I know that the practicing is a diverse activity, in that we all have methods of practicing that suit us best. Knowing this, I'm not seeking for any distinct method of practice, but rather, I'm looking for guidelines or recommendations based on your experiences.
I've been playing piano for three years now, and I've made a strong commitment to stick with piano that I have every intention of fulfilling. In order to do so, I find it necessary that I constantly work to refine and perfect my methods of practice to insure optimal results after each session.
So, without further ado: any suggestions?
What are some methods of practicing (for example, slow practice) that have contributed greatly to the efficiency of your practice?

Practise is a problem-solving exercise. No one method of practising can be said to be universally efficient, in my opinion. Also, the same method of practising might not necessarily work for everyone. The basic tenet of practising, as far as I am concerned, is to identify the problems in your playing, and find a way of practising the particular movement you require for that specific passage/note. This might mean that you are practising a single note from a chord, in order to ensure that your soudn colour is exactly the way you want it, or it might mean that you are practising a long passage in an effort to attain a long line. My best advice to you, is to keep a mental note of what difficulties you are resolving and how, so that you have a better toolkit (so to speak) in the future.

Good question. I think everyone practices differently which depends on their teacher's methods and what is best suited to each individual.
I personally start with finger warm up's. I then move on to the various pieces in my book that I am working on, as you can see I am only 6mnths in but loving every moment.
My teacher told me it's not the quantitiy in time that I practice but the quality. I now play a piece until I have made as little to no mistakes. Then I work on the next piece. I work on the areas I need work on, for me speed and rhythm so I am constantly working on my counting and finger speed.
