Note recognition practice books?

Here's some background information:I started taking lessons when I was about 6 year olds, and I'm 17 now. I took lessons for a few years and then had to quit. I continued to play, but I did not practice scales or theory. Really, I was just trying to play songs rather than master or put in the work to become a better player. Naturally, I hit a plateau and stopped for another few years, frustrated. I recently started playing again, but I'm in a position where I can't have a teacher. I took a look at some of the recommendations and tips from teachers here, and bought; Hanon, Czerny, Burgmuller, the Complete Book of Scales and Arpeggios. I can feel and hear a vast improvement just from starting these books, and the last book helped me reach a breakthrough in music theory.

I remembered that my teacher used to have me play the Dozen a Day Books and bought Level 3 and 4. (I stopped taking lessons way back when I had just reached SMP lvl 3.)Now to the point of this post..

At the beginning of my lessons, my teacher gave me a beginner's music theory book with passages that I would fill in the notes for. This was very helpful to memorizing and recognizing music by sight and feel. I was wondering if there is an intermediate or more advanced version of this method? Currently, I can only recognize two octaves from middle C before I have to manually check what note I need with my fingers. Does anybody know what book series this is or something similar to it? Any other theory books you'd like to recommend would be much appreciated!

Are you talking about reading ledger lines?

There's a face on the top of the staff and another hidden on the bottom. 

Two ledger lines up from the top is C.  Two down on the bottom is also C.
