This is the Dame Myra Hess piano solo arrangement of the Chorale "Jesu bleibet meine Freude" ("Jesu, Joy of Man's Desiring" in the translated English lyrics) from J. S. Bach's Cantata 147.

My first encounter with this piece was about 60 years ago when it was performed on a TV telethon collecting donations for a Jewish organization. When the pianist was introducing the piece, he was reluctant to mention the title of the chorale from a church cantata in front of the largely Jewish audience and said instead "tonight, we will just call it Bach."
