Advance Trills and tremelo issues...

Hi all,
I have beening having problem with trills. (agian!) Oh well... the one that I have seems to be hard ones. Just wonder if anyone can help me out.
1. How to play trills in BOTH hand smoothly and sounds together something like in the end of cadenza the 3rd Beethoven concerto.1st mov)
2. Since trills involves a lot of hand-arm-finger activity, how to seperate the sounds of trills and another voice that comming. (same example as Q1. The end of Cadenza of 3rd concerto where : RH plays D Eb trills and LH plays BC trills and there is antiphonal melodic exchange between RH and LH)
3. How to do the octave trills smoothy. (like in brahms 1st concerto, where we have an octave of Bb alternating with C)
